Grants ranging from $5,000 to $50,000 to Southern Idaho and Utah nonprofit clinics and health care agencies that provide direct medical care and support services to underserved individuals in the Foundation's service area. Funding is intended for programs or services that align with one of the following community health priorities, which includes the prevention of:

-Prediabetes: Improve the prevention, detection, treatment and/or management of prediabetes
-High blood pressure: Improve the prevention, detection, treatment and/or management of high blood pressure
-Depression: Improve the prevention, detection, treatment and/or management of depression, and reduce the suicide rate
-Prescription opioid misuse: Improve the prevention, detection, and treatment of prescription opioid misuse
-Access: Improve access to medical care, behavioral health, or substance use treatment
-Health promotion and wellness: Increase the adoption of healthy behaviors to prevent chronic disease

Applicants must be located in and benefit individuals in the Intermountain area.

The Foundation is particularly interested in funding:

-Innovative projects with measurable results
-Projects that promote significant and lasting change
-Projects that are specific, well-defined and cost-effective

Estimated Size of Grant:

The majority of grants are in the $5,000 to $50,000 range. Over two-thirds are at or under $15,000.

Term of Contract:

Multiyear requests are not funded.

Additional Eligibility Criteria:

The Intermountain Community Care Foundation provides grants to clinics and to agencies that provide direct healthcare and support services to medically underserved individuals throughout the Intermountain area (see Grant Coverage Areas to look up geographic eligibility).

Foundation grants are restricted to non-profit organizations within Intermountain’s service area of Utah and southern Idaho.

Applications must include a letter designating 501(c)3 status.

Not funded:
-Political campaigns, political action committees, lobbyists or organizations with a political agenda
-Fraternal organizations, clubs, school organizations and school athletic funds
-General operating budgets of organizations which receive more than 40 percent of their budget from United Way
-Religious organizations (unless the request is specifically for a program offered to the public on a nondiscriminatory basis and without regard to the recipient’s religious affiliation and does not require worship as a prerequisite for treatment or care)
-Debt retirement or operational deficits
-Trips, tours or travel expenses
-Other foundations or endowments that provide loans, scholarships or donate money they raise to other agencies
-Construction, remodeling or purchase of property, vehicles, or equipment
-Studies, media or advocacy campaigns, research or surveys, and events such as fairs and tournaments
-Services for which third party reimbursement or pharmacy assistance programs are available
-Government entities (local, state or federal, including state-owned schools, public, private or charter schools, and universities)

Pre-Application Information:


-To have your application considered at the Board meeting in May, the application needs to be postmarked by March 1.

-To have your application considered at the Board meeting in November, the application needs to be postmarked by September 1.

Applying agencies need to be aware of the funding timelines for each funding cycle and be prepared should the Foundation Board have questions about their application.

Organizations that receive a grant will not be eligible to apply again for 18 months.
